Description:
· Conduct initial home visit following referral to establish rapport and to plan required services.
· Establish a trusting relationship to conduct child development education, parenting skills, health services and other needed resources identified at assessment or during visits.
· Support families in securing prenatal/child health services, referrals and appointments with other supportive agencies such resource centers, health providers, maternal health,schools and head start.
· Maintain confidentiality of program information obtained from home visits and exchanged while communicating with community and support providers.
· Ongoing conferences with the Supervisor for cases that involve collaboration with other service providers.
· Provide counseling as needed by the families.
· Provide essential services to families to prevent children from coming into care.
· Monitoring the child’s, parent’s and family’s progress.
· Making referrals to community resources as required.
· Follow-up services for children and families referred to the Kanaweyimik Child and Family Services.
· Completing assessments to determine changing needs of children and families referred to the Kanaweyimik.
· Preparation and maintenance of case records which will be documented with contacts, services needed.
· Consultations with other resources and ensuring any crisis assistance are provided.
· Prepare and facilitate group informational sessions and community groups as required.
· Provide transportation for children and families on caseload as defined in the family case plan.
Qualifications:
· Must have a minimum of certificate from a recognized School of Social Work or equivalent training and work experience in the human services field.
· Must have a minimum of three years experience in the human services field and proven ability to work in the human services field, mainly in child protection and family support services.
· Demonstrated knowledge of skills required for families at risk of child abuse/neglect.
· Demonstrated knowledge in the areas of native family systems; family violence; and child welfare services.
· Demonstrated knowledge of the Child and Family Services Act.
· Demonstrated knowledge of the Family systems theory and case management practices.
· Knowledge of skills required for youth t risk of suicide.
· Ability to communicate orally and in writing with all levels of employees as well as the general public.
· Computer skills are a position requirement.
· Excellent verbal, written and time management skills are required.
· A clear criminal records check and vulnerable sectors check must be presented at interview.
· Must have a valid Saskatchewan driver’s licence.
